Now that you have been offered a place at The University of Aberdeen we wish to know about any disabilities or health conditions that may affect you in your training so that, where appropriate, we can advise the University of any support needs you may have and how these can be provided for. We also need to ask about conditions which could pose risk to patients so that we can assess these and consider how any risk can be avoided, whilst assisting you to successfully complete your course. This can be achieved with most health problems and disabilities, even if substantial.

As a potential healthcare practitioner you have a duty to provide relevant information to the University’s Occupational health care provider, GO Health Services (NHS Grampian Occupational Health Service). Failure to disclose information about a physical or mental health problem that could affect patient safety would be a breach of this duty and could result in disciplinary action. All medical and sensitive personal information you provide will be held in confidence by GO Health Services. The School will only be informed of the effects of a health problem or disability, if relevant to your educational needs or patient safety, and of recommendations on support or adjustments that could be of assistance to you.

ONLINE PRE-PLACEMENT HEALTH QUESTIONNAIRE

In order to proceed with your application you are required to provide the following documentation:

  1. A completed online Pre-Placement Health Questionnaire in advance of your first occupational health appointment.
  2. A completed Immunisation Record which has been verified by your GP/Practice Nurse. Please visit the University Fitness to Practise web pages and click on the Immunisation tab for further detailed guidance.

*** Please scan and email this completed immunisation record to gram.ohs@nhs.scot as soon as possible. If you are unable to email us this form in advance it must be taken to your first appointment. ***

If you declare any condition which may require support, or affect fitness for work with patients, an Occupational Health Adviser will contact you for further information.
